How do I add Form 4852 instead of 1099-R?

I took 401(k) distributions in 2024 and might have to file Form 4852 with my taxes if the investment company (Human Interest) doesn't send me a 1099-R. I'm not finding where to add the Form 4852 - will I need to file a paper return?

    Yes. This can be filed electronically if you have all the information correct on a 4852, including the payer's EIN. Without an EIN, the return cannot be filed electronically. Here is how to report.

     

    1. Open or continue your return.
    2. In the Federal section, select Wages & Income.
    3. Scroll to locate Retirement Plans and Social Security.
    4. Select Start or Revisit next to IRA, 401(k), Pension Plan Withdrawals (1099-R)Or Social Security ( SSA 1099, RRB 1099)
    5. Next report the income and information required as if you received the 1099R
    6. After reporting the income in the form, there will be follow-up information to fill out.
    7. One screen that will appear asking you 'Do Any of These Situations Apply to you"
    8. Here is where you indicate you need to file a substitute 1099R, which is the same as a  4852.
