I enter "Nationwide Life and Annuity..." and Turbo Tax shows me "Thrivent".  That's not correct.  How do I get the right one?

You won't be able to import from Nationwide Life and Annuity. It is not a TurboTax Import Partner so you'll need to enter your 1099-R manually

To enter your 1099-R:

  1. Sign into TurboTax.com
  2. Select Continue where I left off
  3. On the left navigation panel, select Federal then Wages & Income.
  4. On the Your income screen, scroll down to IRA, 401(k), Pension Plan Withdrawals (1099-R) and Start/Review.
  5. Answer Yes on the screen, Did you get a 1099-R?
  6. If you land on the screen, Review or Here's your 1099-R info instead, select Add Another 1099-R
  7. Answer any questions until you get to the screen, Let's import your tax info. Scroll down to Change how I enter my form.
  8. On the How would you like to upload your 1099-R? screen, select Type it in myself (which is next to Continue)
  9. Continue answering the questions to enter or review the info from your 1099-R
