I cannot file my tax return because TT keeps demanding the cost at annuity date on from my 1099-R, but this number does not appear on my 1099-R.

The cost of the annuity at the start date is the amount invested in the annuity and refers to after-tax contributions that were made to the plan. If the amount is not reported in box 9b of your 1099-R, you may need to contact your plan administrator or review your financial records to determine your contributions.
