Question

How do I get a duplicate 1099-R form? I have misplaced the original.

The 1099-R is for a 401K withdraw.

You can either logon to your 401(k) providers website and download the statement again if you can, or call your 401(k) provider and as them to mail you another statement. Unfortunately, Turbo Tax does not get or keep personal tax documents.
