How do I get an EIN for a place that has closed down?

Yes, you may call the IRS for assistance at (800) 829-1040. Your employer is required to mail you a copy of form W2 by January 31. If you haven't received it  by February 14, contact the IRS, and they will provide a Substitute W2, which is  reported on Form 4852
