How do I report the additional state taxes I had withheld last year on my return?

I work as a bartended and it doesn't automatically take out state taxes on my paycheck so I filled out an NC-4EZ so that it would take out some money from each of my paychecks.

The additional state taxes withheld will be included on your W-2 you receive from your employer.  Reported in box 17 of the W-2.
