    As a US person ( citizen/GreenCard/ Resident for Tax purposes ) and  for private pension/annuity only you would  pretend as if you had a 1009-R .    That should work.  You probably have to use  Box 1 and Box 2  as same  -- all your distribution is taxable,  Box 7 code  is 7 --" normal distribution".  These  artifacts are just to keep  TurboTax happy and meet the IRS requirement that pension/annuity distribution show up in the correct line on form 1040.
