I am being taxed for an IRA distribution I didn't take

    The IRA distribution would be reported on a Form 1099-R entry. You will find that in the Retirement Plans and Social Security section, which is under Wages and Income. Choose IRA, 401(k), Pension Plan Withdrawals (1099-R):
