I cashed out a small life insurance policy and they sent me a 1099R. How do answer the question"Tell us which 1099-R you have" question? None of the options seems to fit.

Insurance policies/contracts would fall under the first option "Form 1099-R, withdrawal of money from 401 (k) retirement plans, pensions, IRAs, etc."
