I believe you mean you received a total distribution of your retirement money held in a 401-K retirement account. If so, the distribution would be reported on a Form 1099-R and you would report it in the Retirement Plans and Social Security section, then IRA, 401-k, Pension Plan Withdrawals (1099-R) in TurboTax you will find under Wages and Income.
As far as your W-2 form is concerned, the only thing reported on it would be your contributions to the plan during the current year. That would be reported as a reduction in your wages in box 1 and an entry in box 12 referencing code D.
