Solved

I contribute 5% of my wages into an IRA account and 5% in to a Roth IRA. Those distributions are shown in box 12 of my W-2. Do I need to report those in any other spot?

No.  Since the deferrals are reported on your W-2, they are not reported anywhere else on your tax return.

And the accounts you are contributing to are not IRAs.  They are in either a 401(k), 403(b), 457(b) or the federal TSP, none of which are IRAs.
