This will be reported under Other Miscellaneous Income. To get there follow these steps.

Federal Taxes
Wages & Income
Scroll down to Less Common Income
Click Start next to Misc Income, 1099-A, 1099-C
Scroll down and click start next to Other Reportable Income
Click Yes to Any Other Taxable Income
Enter a description of your income and the Amount

No. Because it's an international organization and they have graders from various countries, they do not provide any tax documentation.
