I forgot to record 401k rollover to qualified IRA. Received 1099-R after filing. How do I amend?

IRA is traditional.  Box 2A on 1099-R is marked "taxable amount 0".

You have to amend your 2021 income tax to report Form 1099-R not reported on your original return because you didn't receive it in time, please see  How to amend (change or correct) a return you've already filed.

With a taxable amount of $0, there is probably no need to amend.  Doing so would produce a Form 1040-X that shows that there were no changes.  The only thing that filing Form 1040-X would be to provide an explanation that your originally filed Form 1040 failed to include the gross amount on line 5a and the ROLLOVER indication on line 5b (but no change to the amount on line 5b).  You can always just provide that explanation if the IRS ever questions the rollover, which is relatively unlikely since your Form 5498 from the traditional IRA will show receipt of the rollover.
