I got 2 1099-R forms from rolling over a 401k twice last year. do I need to add both 1099-Rs to my taxes even if I didn't receive cash out?

Yes, you will enter both Form 1099-Rs.  If code in box 7 doesn't indicate the rollover then you can indicate that this was a rollover in the follow-up questions.

 

  1.  Click on "Search" on the top right and type “1099-R”  
  2. Click on “Jump to 1099-R”
