I got an SSA 4926 only; so no box 5.

I did not get an SSA 1099

Form SSA-4926 SM is not the right form for TurboTax. You need Form SSA-1099. 

 

Champ @xmasbaby0 says you can get a copy on the Social Security site, or you can contact the Social Security Administration to request the form. Call 1-800-772-1213 or visit your Social Security office. 

 

Champ @DoninGA  says Form SSA-4926 SM is an informational statement that a Medicare beneficiary will receive when the Medicare monthly premium changes. It is not entered on your federal tax return.

 

My Social Security Form is SSA-4926-SM(1-2015). How can I enter this number?
