I had a Roth IRA but then transferred the entire balance over to a new company. Do I include the transfer or is that counted from the year I made the contribution?

If you rolled assets from one IRA account to another IRA account then you should get a 1099-R form reporting the roll which needs to be entered on the tax return even if none of it will be taxed.  A 1099-R is issued the next January.
