I have 401K. Am I entering that as contribution to traditional IRA?

No.   If you have a 401k through your employer you are entering that when you enter your W-2.  You do not enter it again.

NO.  You do not enter contributions to an IRA if you only have a 401(k) retirement account.

 

Your contributions to the 401(k) are reported on your W-2 in box 12 with a code of D.  They are not entered anywhere else on a tax return.
