I have a 1099-R for a one time litigation settlement - not from a pension or annuity. Can you tell me how to enter this on my tax form?

The payer is Clark ET AL V Duke University ET AL Erisa Lit Settlement Fund.  I received a one time check for $94.  This amount was reported to the IRS and I have a 1099-R.  Since this is not from a pension or annuity, I am unclear how to correctly answer the required questions regarding this money.
