Question

I have an HSA but didn't make any contributions. My employer contributed $300. The interest gained was below $10. Do I need to file? I don't have any tax forms for it.

Yes, if you have to file a return, you have to add the 8889 to report on the HSA.

You must file form 8889 if there were any contributions to your HSA (by you or anyone else) or any distributions (expenses paid) from the HSA.

You already had the tax form needed to do it. It's your W-2. There is probably a code W in box 12 with $300 next to it.
