I have stocks (looking at a statement I did see capital gains / losses and dividend), ira. 401k and Roth 401k do I need to report this if so how and where.

I own a few stocks, I have a work Roth 401k, work 402k, personal IRA

I have seen Capital gains and dividend - do I need to report this if so how and where?

If all these investments are in an IRA or other qualified retirement plan, you do not need to report any of this information to the IRS.  Only report the capital gains/losses and dividends earned in your personal, non-retirement account.
