Question

I haven't received a 1099-R for an early distribution on my 401k yet, but have the amount of the distribution and withholding. Do I need to wait to receive it before filing?

I know the company name, address, total distribution amount and tax withheld.  However, I do not know the federal tax ID.

I would suggest waiting. 1099-Rs were due to be sent out by January 31st so you should receive it soon.
