Please contact the custodian of your 401(k) and inquire about your 1099-R. It may have been delayed or misdirected.
By law, you must have the 1099-R in hand to file your return. The IRS receives a copy of your 1099-R and will compare it with the entry on your return. If they do not match, you will receive a letter from the IRS, possibly with a bill for underpaid taxes, penalties and interest.
