Question

I haven't received my 1099-R after withdrawing from my 401k, I don't think I will be able to get it before the tax deadline. What can I do about this to avoid penalties?

You can create a substitute. Click on the "What if I don't have my 1099-R" at the bottom of the screen.

 

Form 4852 serves as a substitute for Forms W-2, W-2c, and 1099-R and is completed by taxpayers or their representatives when:

  • Their employer or payer does not give them a Form W-2 or Form 1099-R, or
  • An employer or payer has issued an incorrect Form W-2 or Form 1099-R

You may be able to pull it from the 401(k) administrator's website.  Or, get a Wage and Income transcript here:
