I left my last job, the 401k contribution was distributed to me by Fidelity since the balance is below 1000, but I didn't find the tax form when i logged into my Fidelity

because distribution is only $64.55, I'm not sure if I should contact Fidelity for a tax form. and how do i report this distribution

Contact the plan administrator for a copy for the 1099-R that is required for a distribution of $10 or more per the IRS instructions.

 

Specific Instructions for Form 1099-R
File Form 1099-R, Distributions From Pensions, Annuities,
Retirement or Profit-Sharing Plans, IRAs, Insurance
Contracts, etc., for each person to whom you have made
a designated distribution or are treated as having made a
distribution of $10 or more from profit-sharing or
retirement plans, any individual retirement arrangements
(IRAs), annuities, pensions, insurance contracts, survivor
income benefit plans, permanent and total disability
payments under life insurance contracts, charitable gift
annuities, etc.
