You must have been a resident of Michigan at some time.   The CF-1040 is a Michigan city common form for the cities of Albion, Grayling, Lansing, Portland, Battle Creek, Hamtramck, Lapeer, Saginaw, Big Rapids, Highland Park, Muskegon, Springfield, Flint, Ionia, Muskegon Heights, Walker, Grand Rapids Jackson and Pontiac.  This common form is used by all the cities mentioned for taxpayers who live or work there to report income, exemptions etc. The form may be triggered in TurboTax if you have a W2, 1099 or other income document that has an employer/vendor using an address in any of those cities.

You can change this by going through the state section of TurboTax and continue until you get to the Live or Work in Any of These Cities? screen and uncheck the city that is being generated.

If you did not live in Michigan in 2020, go back to your "My Info" section to review if you indicated you earned income in Michigan or Michigan is your resident state.
