I only show a box 10 state income tax withheld, no box 11 or box 10a, 10b

If you are entering unemployment income, it is more important to be sure that you are entering your information based on the descriptions on your form matching the descriptions on the screen instead of matching the box numbers.  Not all Form 1099-Gs issued by different states conform to the same box numbers.  

Be aware that since your form is showing that there were state taxes withheld, you will need to enter information in boxes 10a, 10b, and 11 on the TurboTax screen in order to avoid an error condition.  Box 10a is the state name and box 11 is the state taxes withheld.  If your Form 1099-G does not show a state ID number for box 10b, then enter the Federal ID number shown on your form.
