I payed more than 18K into my 401K due to change in employment (18K with my Fidelity 401K and 6K with my Vanguard 401K). Now it's too late to withdraw it (Apr 16th) What forms should I feel this and next year?

You always have until the due date of your tax return to withdrawal this amount. This year, you have until April 18, 2017.
