I receive social security. Do I have to report that?

I don't have my statement, but I know how much I received. Do I need to report it on this income tax form?

Yes, you do need to report your Social Security income on your tax return because it may be at least partially taxable due to other income on your return.

 

 

You should also try to get a replacement copy of your Form SSA-1099 to report the income.  There may have been deductions from your Social Security income for Medicare or other insurance costs.
