I received a 1099-r in the mail. I had a 401k with the previous employer which rolled it over to an ira. Since I received no income or distribution, do I still enter this

Yes, you need to enter it to document the rollover. If you do not document the rollover, the IRS is going to assume you have the cash and will delay processing your return because they only know that money came out of the 401K, they do not know where it went.
