Question

I received a check not a form about an annuity account that was closed. The 10 percent tax penalty was not taken out. Where do I place this income under?

It depends. I would wait to receive the 1099-R form before trying to report this on your own. If this is a check you received in 2020, you may wish to contact the issuer to see if they will issue a 1099-R. If you received your money in 2021, you won't see your 1099-R until next year. 

 

In any case, if taxes were withheld from the distribution, the 1099-R will indicate this in Box 4 of the form.
