Question

I received a Statement of Payments from one of my employees instead of a W-2. How do I report this income on Turbo Tax?

This Statement of Payments has "FORM 1099-MISC" written at the bottom right hand cover, but does not look like the form 1099-MISC that Turbo Tax provides (mine has no boxes, but it does include the income I earned.
