I received long term disability benefits but did not receive any tax forms. Do I need to enter this information somewhere?

It was a benefit I received from my employer and the benefits were paid by a third party.

That depends on how you paid the premiums for the disability coverage. If the employer shared in the cost for the premiums, then yes, the disability payment to you is taxable. The payments (claim payments) will be taxable based on the proportion the employer paid towards your premiums, and the portion paid by you if you used after tax payroll deductions. If you paid for the premiums entirely by yourself using after-tax dollars, then the payments (claim payments) are not taxable

Long Term disability is taxable if the premium was paid by your employer or paid by you with before tax income. If you paid the disability premium with after tax dollars (no tax deduction), the the disability income is tax free. Contact the third party and ask what information they have and confirm whether the payment is taxable or not. If this was an employer provided benefit, ask them how the premiums were paid. The third party provider should send you a w-2 if the payments were taxable. Hope this helps and LMK how it goes.
