Question

I received my 1099-G form and for my State ID on my form is blank. MI taxes unemployment and i cant put it through without it.

I have to add my State Identification number and cannot do so because the form i received is blank. How do i find the number now?

You will have to contact the MI Department of Labor Unemployment Office to ask their employer number.  Their contact information can be found here:  Contact UIA

 

In the meantime you can enter the Federal EIN in the field for the MI number.
