I received two 1099-R forms at the same time. They both have different company's on them but one says substitute on the bottom. Which one do I use?

"Substitute" simply means that they are not using the official IRS printed form.

Both I would assume ... did you receive distributions from both companies ?  Did you get all the money they said you got ?
