I received two 1099-r forms from different companies and both forms of the same exact amounts indicated in box 1 and 2a. What do I need to do to?

do I need to input information from both identical 1099-R forms despite from different institutions from BNY Mellon Dispursement Agent and Prudential Tax Services LLC?

Probably.  Why did you get 2?  Did you roll over or convert the money?  Or ask them why you got the duplicate 1099R forms.   Is the code in box 7 the same?  And any withholding?
