If I converted my 401k into an annuity, do I have to put that on my tax form, and if so, where?

You would need to report that if a Form 1099-R was issued. If the annuity is a tax-favored account then the transfer would not be taxable though. You would enter the Form 1099-R in the Retirements and Investments section, then Retirement Plans and Social Security, then IRA, 401-K, Pension Plan Withdrawals (1099-R)
