If I received a 401k distribution check of just $140.14, should I expect a tax form for that?

It was in an account from a past employer, where I no longer work. Because of the small amount, I did not roll over.

    Yes, you will receive a 1099-R for this distribution. If you have moved, they may have sent it to your previous address. If you have an online account with the company the distribution came from, you can check there for the tax form.
