If IRA Rollover reporting on Form 1009-R, should Box 7 indicate G and the IRA/SEP box be blank?

Box 1 is $11,664 but 2a is zero, and box 7 is "G", IRA box is empty.

Yes, if this was a direct rollover from a 401(k) to a traditional IRA, then this is correct.

As DanaB27 indicated, this was a direct rollover from a 401(a), 401(k) 403(b) governmental 457(b) or the federal TSP, not a rollover from an IRA, correctly reported.
