I have a form 1099-R, Ira to Roth conversion. Since I had more income then expected in 2017, I reversed the conversion. Do I still need to file the 1099-R and if yes, how to I indicate the reversal?

Yes, you will need to file the 1099-R. 

However, there will be a question after entering that asks what you did with the funds.  Your answer is that you rolled the funds to a Roth account.

The next page, Move Funds?, will ask you if moved any part of the money back to the IRA.  And the following page asks how much.
