I received a "2017 Required Minimum Distribution Statement.  Is it the same as a 1099-R?

Not necessarily. This is usually a mailing from your custodian telling you that you need to make a Required Minimum Distribution.

If you thereafter took a Required Minimum Distribution from your account, then you should have received a 1099-R.

Does your statement have these box #s on it even if it's not laid out the same way?
