Is box 1 box 3 and box 5 supposed to show the same amount on my W-2

No, not necessarily. 

 

If you had any pre-tax employee contributions, such as to a retirement plan. 401(k), or Flexible Spending Account, the amount of TAXABLE income subject to Federal Income Tax would be less than the amount subject to Social Security Tax.
