Is there a problem with accessing Form 8915- E? I have entered all information needed for a 4011k withdrawal and have answered questions about COVID-19.

Turbo Tax website says " We'll complete and include Form 8915-E with your return if you qualify. Just enter your Form 1099-R and answer a few questions to determine your eligibility" However the system is not asking me if I would like to pay it over the three years.

Correct, the program asks if the distribution was due to COVID and then spreads it out when you answer such that you are eligible.

See How do I preview my TurboTax Online return before filing? to see that 1/3 is listed in your income rather than the full amount.
