Question

It's asking me for Monthly Premium Amount based on my 1095-A form. The form does not show this, so should I just use the contributions that I pay from my paycheck?

The Monthly Premium Amount should come from Part III column A.  If you just have an annual total on line 33, it would be that amount divided by the number of months covered.

 

The amounts that come from your paycheck will not be correct if you have any advance premium tax credit covering part of your premiums.
