Minimum distribution I took out not reported to me

I withdrew my minimum distribution in December but my financial institution did not send me a 1099.  How do I correct this

if you have online access to your retirement account, you should be able to download a copy of your form 1099-R.

 

Otherwise, you may reach out to your broker for a copy of the form 1099-R they sent you as it may have been lost in the mail.

They did not send me one.  I withdrew amount on Dec 28.  I have the confirmation paperwork, but they did not transfer the money until Jan 3.  (that is when it hit my bank)

Is this your first Required Minimum Distribution(RMD)? 

If it is, you have the option to take your first RMD by December 31st of the year you turn 73 or wait until April 1st of the following year.
