My 1099-R form does not have a box 5 or 12 that is asked for on my return. Also some of the other boxes do not have the same label as my State of Oregon 1099-R

Just use the boxes that are on your form and leave any other box blank. Some payers dont put boxed on their form that does not apply to them but TurboTax must have all boxes.
