My 1099-R has IRA/SEP/SIMPLE on it, should I mark IRA SEP or IRA SIMPLE on my tax return?

My 1099-R has IRA/SEP/SIMPLE on it, should I mark IRA SEP or IRA SIMPLE on my tax return?

    Only if that box has a checkmark in it like an X.  What plan is the 1099R from?

    When you enter your Form 1099-R, you mark the IRA/SEP/SIMPLE box since the IRA/SEP/SIMPLE box is checked on your Form.  You're not going to actually see the box on your tax return.
