My 1099r has code 4g in box 7 and it should only be a g I have tried getting a corrected one with no luck it will not affect my taxes can I file it with a g in box ?

    You must ask the 401(k) plan to issue a corrected code 4G Form 1099-R showing that $0 was distributed to the beneficiary of a deceased participants 401(k).  They then need to issue a correct original code G Form 1099-R for this distribution to the obviously alive participant.  If they refuse, you'll need to enter a substitute code G Form 1099-R and provide explanation.

    Code 4 is for a death and code G is a direct rollover....was this due to a death?

    • Click on Wages and Income (Personal Income using Home and Business)
    • Click on I'll choose what I work on (if shown)

    Not a death I just rolled over my personal 401 from  former employers to an ira

    Yes, I would file that with code "G."
