Question

My 401k allows me to contribute with a Roth Ira as well as traditional. Is there a separate form i should get that breaks it down on my taxes?

    An IRA is not a workplace plan.  An IRA is an individual account that you set up at a bank or broker.  I think you mean that your 401k has both a pre-tax option and a Roth option account.  All your workplace contributions should be reported on your W-2 in box 12 with different letter codes.  You do not enter them anywhere else on your tax return.
