My uc 1099g form from pa doesn't have a box 1, box 4, or 11. what do i put in for turbo tax?

PA only has 4 boxes and it's for ssn, total payment, taxes withheld and address.  where do i put this information when filing?

Try matching the information on your form to the description of the boxes on the TurboTax screen.  The format for reporting unemployment compensation is not consistent from state to state and the information may not match the TurboTax screen exactly.  

 

The important thing to note regarding boxes 10a, 10b, and 11 is that if you did not have any state taxes withheld, then be sure to leave all three of these boxes blank. 

 

But, if state taxes were withheld, then enter the amount of state taxes that were withheld in box 11 on the TurboTax screen.  Use the dropdown selection to fill in box 10a with your state name.  Enter the Federal ID number (or Federal TIN) from your form for box 10b if there is no state ID number included with your information.  

 

Additionally, if your form is reporting both unemployment compensation and paid family leave, then you should not add these together.  They should be entered as two separate Form 1099-Gs.

 

To be sure you are in the right place in TurboTax to enter your unemployment information, use these steps:

  • On the top row of the TurboTax online screen, click on Search (or for CD/downloaded TurboTax locate the search box in the upper right corner)
  • This opens a box where you can type in “unemployment” and click the magnifying glass (or for CD/downloaded TurboTax, click Find)
  • The search results will give you an option to “Jump to unemployment
  • Click on the blue “Jump to unemployment” link
