Question

Non-Qualify IRA rollover by the recipient to a Credit Union. Received a 1099R form. Must I report it to the IRS when I file.

I have forgotten to specify I have deposited the Rollover I had to withdraw from an Insurance company to an  IRA CD.

Yes, if you received Form 1099-R, you must enter it on your tax return.
